In 2024, Medi-Cal health plans must meet new requirements for quality, access, accountability, and transparency in order to improve them statewide.

If you have Medi-Cal and live in one of these counties – Butte, Colusa, Glenn, Nevada, Placer, Plumas, Sierra, Sutter, Tehama, and Yuba – your healthcare plan changed as of January 1, 2024. Counties in blue were previously included in the Partnership HealthPlan.

On January 1, 2024, the counties listed above were automatically enrolled in Partnership HealthPlan. Your benefits, which include primary and specialty care, as well as hospital services, will be managed by Partnership.

Seneca Healthcare District and the Lake Almanor Clinic are Providers with Partnership. This means individuals with Partnership can utilize services at both the Lake Almanor Clinic and Seneca Hospital, even if they do not live in Plumas County.

Any individual with Partnership can be seen at Seneca Healthcare District and/or the Lake Almanor Clinic regardless of the Primary Care Provider (PCP) listed . If you need a specialty referral, and Lake Almanor Clinic is not the PCP, Partnership will not cover it. You will need to see the PCP listed on the front of your insurance card for the referral. If the provider you are seeing at the Clinic is not your PCP, you can complete the PCP Selection Form available at reception in the Lake Almanor Clinic or on our website at www.senecahospital.org/pcpselectionform.

When completing the form please ensure to list LAKE ALMANOR CLINIC as your provider with Tax ID# 94-1322200 and Phone 833.227.3743. One form must be completed for each patient.
