If you have any questions regarding our hospital’s scope of service or appropriate clinical privileges, please contact our office. Please sign and date all of the supplemental forms where indicated and attach all requested supporting documentation. Upon completion, forward your application packet along with the application fee to Seneca Healthcare Districts Medical Staff Office at the address noted to the right. Please download the appropriate applications below to apply for privileges and membership at Seneca Healthcare District.

Application Instructions

  1. Download the applicable application form.
  2. Fill out the form on your computer and save the completed document to your computer.
  3. Print and mail the completed documents along with your curriculum vitiate (CV) and all requested attachments

Processing Time for Completed Applications

Our average processing time is 60-90 days following receipt of a completed application. We ask that you pay special attention to the instructions included in your application packet, as Seneca Healthcare District cannot move forward with the credentialing process until the completed application has been received.
